I use the standard layout and other Orca keys such as Insert+Z or
Insert-Q work.
I just tried Insert+Control+Space and Control+Insert+Space several times
on my various keyboards and it worked fine for me. I'm not sure what
might be going wrong on your machine. What kind of keyboard do you have?

What's the problem exactly? You've already said that XUL applications
shouldn't use keys without modifiers. Are those keys (together with
their Shifted variants) together with Orca modifier keys and some arrow
combinations not enough for Orca commands?
As I mentioned below, the Orca user base has requested that we not
require the user to press the Orca modifier key in combination with the
given keys. We are honoring their request.
