Hi Vsevolod.
When NumLock is on, it is expected for the numpad keys to work like a
numpad. To have them work for Orca commands, NumLock should be off.

Yes, that's true. I was alt tabbing and also another keystroke I
pressed
was ctrl+alt+d to go to the desktop where NumLock works.

Yes, that was after I went to desktop, then opened a new terminal
window
where flat review worked.
Did you actually give a bunch of flat review commands rather than
just
a few? If that's the case, then my guess is that the problem is the
app
in question is no longer emitting accessible keyboard events. A way
to
test that would be to try other Orca commands at the moment flat
review
is failing. For instance, does Orca+H put you in help mode if you
press
it immediately after flat review fails?
If I press insert on a NumPad, (NumPad 0 key) it doesn't work.
It enters 0 and h.
If the regular insert key is pressed, the help is opened.
