This thing is now solved for NVDA on Windows.
Jamie has came up with a clever trick on how to go about fixing
it.
I guess something similar might also be doable within orca..

Firefox already provides a relation to get the tab document for a
given accessible, as well as a relation to get the currently
active document from the root. We could use those to fix this.
BTW what's that special live region suitable for instant messaging
apps?
