I've sorted it.  In my gnome preferences keyboard layout there were 2
layouts listed, US and UK, with the UK one as the deffault.  I've
deleted the US one as I never use it, and things seem to be fine now.
I suspect that every time I switched to gnome the layout set off as US
and was then imediately switched to UK, thus resetting xmodmap.  

Anyway, I thought I'd share this in case anyone else experiences the
same thing.  Thanks Will for suggesting I check xmodmap.

That's exactly what's happening.  At least its not an Orca problem;
I'll have to try and track down why its being reset.
