I do not have access to this document.  When I login to my Google
account (brian art cameron gmail com), it says "We're sorry, but
brian art cameron gmail com does not have access to this document."

If you give me access, I can review it.

Some minor comments:

The above is in the past tense, which makes it sound like the person
has already given the talk.  However it was my understanding that the
form should be filled out before the event.  If so, the following text
might be more clear:

"We are looking forward to having you speak at COSCUP / GNOME.Asia 2010."

Using the word "and" multiple times in a single sentence is a bit
awkward.  It would sound better to say: "In order to promote sharing
information about Free and Open Source Software, please sign this
form..."

I would say:

in order to release any recordings of your participation at the event.
All recordings will be released with the "Creative Common -

Instead of "under the license of..." I would say:

I'd think the "August, 2010" should be removed.
