First thing is a big thanks for bringing this up here.
Second thing is it's OT, so if we are g going to continue the discussion we should probably take it to 
another space.
I offer the open-sourceaccessible google group for this, but am open to other spots as well, prefferably 
distro agnostic.
As for the message here to be clear with speech dispatcher the only voice options that can be controled from 
yasr are rate, volume and pitch, based on 
speechdispatcher's -100 tl 100 range with 0 being 50% of any of these.
There are a few other things that control what is spoken, and a dictionary in the config file that is only 
used for punctuation, but may be usable for 
words as well/going to try after work.
Key echo is off by default, but can be set to words or letters, keysactually I think it's called, and there 
is a cursor tracking setting that is set to 
full, which is probably the best for most aps, but like speakup's different modes sometimes an alternative 
mode is called for.
Be careful with that synth choice item. You'll have to restart everything if youmess with it and yasr goes 
silent.

When I select "bg" in yasr options-> syntesizer options -> Language,
espeak starts using the Bulgarian voice. Those are hard coded - that's
why you don't see other languages.
Trying to change the Module option in the same "dialog" (this should be
speech-dispatcher's) does nothing, but I don't really have other
synthesizers than ESpeak, i.e. I'm not sure if it doesn't work or it
just falls back to ESpeak.
If I try to change the Sinthesizer option in the main options dialog,
yasr becomes silent. Those should be the build-in modules, so again - I
don't have other synthesizers, but been silent at least indicates, that
this actually does something, though it might be not the right thing, grin.
When you're happy with the current configuration, don't forget to save
it with Control+Alt+s.

It will only allow you to change speech rate, pitch and volume. At least
I haven't found a way.

default synth for a given language, and if you do this I'd think yasr
would use

Make sure the synths are uncommented of course, but I reckon this is
done or you'd not be able to use the synths with orca or where ever
you are using
them now.
