My problem seems to be like Thomas's. I tried yet again to change the setting in voices from "default" to "uppercase". I hit the "apply" button, then the "OK" button. I then reentered preferences and found the setting was still at "default". Is there something else I should do?

O.K. Mind telling me how you got that working. I'm running Ubuntu
12.04 with Orca 3.4.2 and ESpeak doesn't increase the pitch with
capital letters at all. It makes no difference between F and f. It
treats both as lowercase regardless.

earlier install with a work around, but this is a relatively new
install, and I haven't found what that work around was yet to get the
pitch increase from getting stuck at the higher pitch.

Hmmm...Haven't seen that problem. Of course my copy of ESpeak never
increases pitch at all when seeing a capital letter so hasn't gotten
stuck. :D
