This bug seems to have been with us a long time, and was a topic on the
accessiblefreedom mumble server chat lately.

In terminal, use
orca -t
to redo the settings. When orca restarts, go to preferences and set the
upper-case pitch to something you like. Hereafter, orca will raise the
pitch for upper case, and lower it again.
