I had one of these crashes yesterday afternoon and I captured some debug
logs and filed them with a bug; sorry, I forget the number off top of my
head but an easy to repro is to open a gnome terminal and start hitting
the keypad enter key. Very shortly for me after that, I completely lost
speech. Hopefully the debug outs will help. JoanMarie was fortunately
able to repro it on her Open Solaris machine.
I really hope that some of these speech loss fixes will reach over into
other scenarios as well.

These hangs/crashes really need to be fixed, so the more information you can
collect the easier it will be for our hard-working, dedicated, highly capable
and experienced Orca developers to track down the causes.
I haven't experienced this myself recently, but then I don't use Orca a lot -
mostly for accessing Web pages that require Javascript and modern Web APIs.
