Disclaimer: I did my best to find an answer to this in the documentation
and elsewhere but did not find anything relevant.

I'll bet you could make new folders by by case number and create those folders using
procmail plus a simple shellscript, Python program, or if lots and lots of email, a
C program. Procmail has a way to transfer the mail to an external program for
processing before the email's ultimate disposition.
