Oh, I have found easier steps:
* Open any conversation in element
* Make sure you are in browse mode
* Read from up to bottom and stop on a message which is not showing reactions toolbar.
* Now move mouse pointer to that message pressing orca+numpad slash.
* And finally continue navigating line by line forward.
The issue is that orca can't continue reading past the message actions toolbar.
The rationale behind such a seemingly unusual implementation is
that message actions toolbar only appears when hovering with the
mouse or trying to focus that part of the timeline with the
keyboard e.g. navigating with tab or shift+tab.

Sorry for the delay testing the fix.
Unfortunatelly it has not addressed occassional inability to navigate forward by line within the element timeline.
Here are some rough steps:
* Login to any instance of Element such as https://app.element.io/ or https://chat.igalia.com/ in Firefox with orca running.
* Press ctrl+k to jump to the filter rooms entry switching orca to focus mode at the same time.
* Continue navigating to the room list by pressing down arrow key.
* Find a direct chat or room with a lot of messages use right arrow to expand room list sub trees people or rooms if needed and activate by pressing enter key.
* You should land into message composer with orca in focus mode.
* Now make sure your window is not maximized or it does not contain the whole conversation. What we need is that the timeline has to scroll in order to feel the issue.
* Now press up arrow key to scroll the timeline displaying older and older messages. Orca is not giving feetback at this point and I think it's okay at least I don't know how to make this better.
* Now switch orca into browse mode, then press shift+m to jump to the navigation followed by m to jump to the content.
* You should now be reading room header where the room name, room topic and call buttons are located with orca in browse mode. Continue reading forward line by line pressing down arrow key reading timeline content until the timeline has to scroll.
Actual results:
Orca browse mode position very often appears to be stuck somewhere in the message actions toolbar not being able to scroll the view down and continue reading further. The workaround is to navigate to the next link by pressing letter k.
Expected result:
Browse mode navigation should continue scrolling the timeline until there is indeed something to read.
Please note I am afraid there might be other places where this might be happening. Usually I am not too picky and I am trying to find a workaround for my own usage.
However here my position in relation to the element web is a bit different as I have contributed a few little patches to that app and I am often one where people are asking if there is a way to improve the experience while reading messages timeline.
I would appreciate if you can have a look at this and even if there is nothing you can do about it perhaps you will be able to describe something that should be improved within the element.

I just committed a theoretical fix based on your description. The
recovery logic wasn't getting called for children removed from live
regions in order to be performant. Chatty live regions and lead to many
children removed events which are irrelevant.
Hopefully my theoretical fix is still performant and maybe even solves
your issue. If not, then I do need concrete steps to reproduce the
problem.
