I don't know what you could do but I reproduce the same issue on my
computer with Debian 8 and Orca 3.24.
Maybe try "pulseaudio -k" to force PulseAudio to restart?
Or alternatively try killing Speech Dispatcher? I'm pretty sure it will
respawn automatically the next time Orca needs to say something.
