[orca-list] possible minor bug: orca sometimes does not respond to the alt+super+s shortcut

This is another bug that's intermittent  and hard to track down.
Occasionally when I want to toggle orca off and on, eitehr because of
a freeze or because I've upgraded orca, the alt+super+s shortcut fails
to work. Instead orca simply reports whatever app I'm focused on. To
get the shortcut working again, for a little while at least, I need to
run killall orca, then press the shortcut twice, once to turn orca
off, then turn it back on. After this is done, orca will continue to
work with the shortcut, for a while. I know I'm being vague and I
can't give exact steps to reproduce, but I'm hoping someone can narrow
down a set of steps that will reliably reproduce it.
