Ah, I hadn't noticed that the Attachments submenu had moved from the
File pulldown over to the Message pulldown.
You can also tab to the end of the message until you get to the
attachment link and then either press enter on that link or use the
context menu to get to the save dialog.

If you have specific questions maybe I or others can help, but I'm
not aware of a getting started or how to for a blind user and

Basically, you have your list of accounts, which you can arrow
through using up and down arrow keys. When you're on the account
who's mail you want to read, tab over once to get to your list of
folders and again you can up and down arrow to move through the list
and use left and right arrows to open and close sub folders. When you
get to the folder you want, such as your In Box, tab over once to get
into your list of messages in that folder. There's a Mozilla bug
where this list will get corrupt over time, so you want to be careful
to check your message before moving or deleting anything.
I just use the preview pane to read my messages, and the F6 key will
take you to the top of the message text from the list of messages.
Orca+; (That's a semi-colon.) will read the rest of the message, but
you can also use the arrow keys to navigate around the message.
Your standard Thunderbird short cut keys will work, so when you're in
a message you can delete it with the delete key, move to the next
message with the 'f' key and move to the next unread message with the
'n' key. Sometimes you need to use Orca+backspace to pass these keys
through Orca into Thunderbird.
