This sounds very much like the problem I am having, orca just gives no
feedback on what you do in the window until you navigate back to the
window (as you said orca seems to be unaware of the window becoming active).
I will add comments to the mozilla bug (if I feel I can add anything).

Hi all.

The difference when alt+f6 is pressed and when it isn't pressed is that
orca doesn't seem to recognise that the dialog has got focus. By this I

speak or braille anything even though the dialog has focus and actions
may be performed on it.
That to me sounds like this Mozilla bug which I filed a couple of months
ago: https://bugzilla.mozilla.org/show_bug.cgi?id=466332
In particular note what I said when I "pinged" the bug last week:
Ping. :-)
The impact of this bug for the user is that when you get into and then move
around within the Preferences dialog, you have no speech and no braille because
we get no events letting us know that this window has been activated. While one
can Alt+Tab out of and back into the dialog as a workaround, it's hard to know
that this needs doing if you can't see what's on the screen. :-)
It would, therefore, be awesome if this could be addressed sooner rather than
later. Thanks much in advance!!!
Does this sound like the same issue you are reporting?
Thanks!
