In Ubuntu Karmic, I bind Shift+Ctrl+O to:

   sh -c "killall speech-dispatcher; orca --replace"

If speech-dispatcher hangs, orca stops speaking and you have to
restart both to get orca running again.  There are other causes for
crashes, though, so now and then I have to log out and in.
