I also get this being placed back at the top of a page when going back.
I am using orca 2.27.5 package from debian experimental repository. The
rest of my system is GRML (using debian unstable for debian packages,
such as gnome).
As I remember this issue has been present on my system for sometime
(certainly back in the 2.26 versions of orca).
I think it is for all pages. Here is an example where I do get it (just
tried it) and I don't think it is to do with the page doing unusual
things with focus. Go to the orca wiki http://live.gnome.org/Orca and
select the accessible applications link. Now read some of the page and
then press alt+left cursor to go back. I find I get placed at the top of
the page.
Would a debug log file be useful?

A fix was put into Orca quite some time ago. I just tried it and it is
still working for me.
Note that some sites do things to the focused object when it is
activated (like remove focus from it). Other sites change focus when a
page is (re)loaded. We have no control over those things and do not
attempt to override the default behavior of such sites.
Are you finding this issue occurs all the time, or just on some pages?
