This is indeed interesting. I'm not quite sure how to do this -- we never really thought to provide hooks for this kind of thing. For braille, you might try goofing with default.py:updateBraille. I'm not sure if you can replace/redirect it by importing orca.default in your ~/.orca/orca-customizations.py file, but it might be interesting to try as an experiment.
