Could you

1) stop NetworkManager 
2) kill any running wpa_supplicant
3) rmmod ath_pci
4) modprobe ath_pci
5) 80211debug -i wlan0 +dot1x +dot1xsm +wpa +assoc +auth
6) athdebug -i wlan0 +state
7) start NetworkManager, try to associate
8) kill NetworkManager
9) kill wpa_supplicant
10) rmmod ath_pci

Mail me the output of 'dmesg' and the BSSID of the AP you are trying to
associate to for either WPA or open networks.  Please also note your
madwifi version (0.9.3.3 or whatever).

In my testing, there is no difference between the wpa_supplicant output
of an NM driven association and a madwifi association.  Also make sure
you leave ~2 or 3 minutes between association attempts, as some APs can
get into a state where they think you're already associated.
