Does that need to be in the dialog though?
You could for example have "Connect To Server" as a submenu in Nautilus.
This submenu would contain a list of previously connected servers (or
favourites) and a "Connect To New Server..." menu item at the bottom
(which brings up the dialog).
That way we can easily stop the dialog becoming more complicated.
