Usually they are the same people, or have several common members.

I mean, there is no real point in having per-projet separate translation
teams; there should be a big coordination team per language, and then
it could organize work as needed for translating various programs
and projects.
Don't see it as "team: Gnome, sub-team: vietnamese translation",
but as "team: vietnamese translation, sub-team: Gnome".

Indeed a lot (nearly everything in fact) is common to any desktop
(not only Gnome and KDE), the same concepts and words are used;
in fact the same translation memory could (should) be shared
 
You should try to contact all people and teams working on vietnamese
translations of free software, and try to get something built together,
something independent of any particular goal at a first time, the
main goal should be "discuss about how better use our language into
the computing environment", then with those guidelines actual translations
are just a matter of applying them.
But the general discussion could (should) involve people that aren't
translating and don't even plan to translate anything; simple users,
scholars of the language maybe, etc. If peopl doing translations of
technical manuals about computing, or computing magazines, etc, could
be involved, it would be even better.

From what I read in this thread, your main problem is not that much a
lack of coordination in the translating itself, but a lack of common
lexicon. Once a common wording is devised, then even without much
coordination between translators, the texts are much more homogene
