it sounds like [1]. The evolution-source-registry process is dealing
with calendar and book sources of evolution-ews. Once it decides the
calendar is missing it'll remove it from the list. I guess the process
is unable to connect to the server, thus it removes the source. It can
also be due to connection (VPN) not being available on session start.
This can be partly related to [2]. Basically restarting all evolution
processes (ps ax | grep evolution) should help, it's only a bit tricky
when you've calendar factory autostarted (can be by gnome-shell), to
make sure that the calendar factory is running against the newly started
source registry.

Out of curiosity, did you have the whole system running for couple days,
or you logged out and in during that time, not only closed evolution?
