As Patrick says, it really depends on what sort of account we are
talking about - is it local mail only, IMAP, POP, Exchange etc.  Are all
200,000 (!) deleted messages in one folder?  If they are in separate
folders, can you visit each folder and expunge the folder? (Folder ->
Expunge)

I would seriously advise that you turn on View -> Show Deleted Messages
as well so you can see properly what's happening.
