[evince] Ink Annotations and Appearances

I am trying to implement ink annotations in Evince.

To write the appearance stream, would it be advisable to

(a) re-implement PDF drawing primitives

(b) extend Cairo so that a "PDF stream surface" exists? Currently it
is only possible to output an entire PDF file, but that is too much
for an appearance steam.

Anybody with ideas?
