[Replying to my own mistake.]

Both evince and poppler handle the link destinations right. The PDF
documents are wrong generated.

/FitH requires the top value. /XYZ requires left and top values.

The PDF document is wrong with /FitH because it takes the left value as
top value. This explains the apparent link destination misplacement.
