I see some problems with poppler master after a quick check of
KylesFormFillableSR5Sheet.pdf.

The form fields seem to be in the right place, either with poppler
0.18.4 or master.  Please, notice that some fonts are not embedded in
the document.  Some rendering issues or misplaced fields could be
because of that (e.g if you do not have installed Arial or ArialMT, or
an equivalent font with the same metric).

There are some issues in poppler, as I see the output in a terminal:

Syntax Warning: Could not parse ligature component "cyrillic" of
"cyrillic_otmark" in parseCharName
Syntax Warning: Could not parse ligature component "otmark" of
"cyrillic_otmark" in parseCharName

I think those are used in the section "Condition Monitor", when you
click on any of the tables "Physical Damage Track" or "Stun Damage
Track", or in the field "Edge Points" under the section "Attributes".

Regarding to the text fields, after pressing Enter, the input entered is
not shown, but the content is there.  Some of them are shown after
filling other fields.  IIRC, there is a bug report that describes the
same behaviour.
