There is no account to set up.

In the address field of Ekiga, simply type "sip:user@192.0.2.1" where
user is anything you like (it would be important if you were talking to
a sip router - ekiga ignores it) and 192.0.2.1 is the IP of the person
you wish to talk to.  If you have name service that maps a name to the
IP, you can use the name instead.

If either user is behind NAT, then things get complex, and you should
probably use ekiga.net.

I would advise you to use IP6 to avoid NAT issues (signing up with a
tunnel broker if needed), but sadly, ekiga does not seem to support
IP6.  I've tried sip:user@2001:db8::1 and sip:user@[2001:db8::1]. 
Neither works.

The other thing to do to get around NAT is to set up a VPN.   See
http://openvpn.net which can traverse NAT, and runs on Mac, Linux, and
Windows.   Set up a machine that is on most of the time and has a stable
IP (e.g. static or cable) as the VPN server.  Note that diamondcard.us
offers a VPN service, but I think it is only between you and diamond card.
