I agree that there should be a keyboard combo that lets you close a
badly-behaving program.

I don't know that necessarily means opening System Monitor. System
Monitor is quite large and slow, and it doesn't float on top of all
other windows (it would be annoying if it did). The interface for
closing a badly-behaving program should be small, it should appear
instantly, and it should float above all other windows, making it harder
for a badly-behaving program to hide it.

I think making the keyboard combo Ctrl Alt Delete would be good for
consistency with Windows, but bad for marketing.
