Oh, and if this is not an one-timer, make the custom extensions
entered to be added to the original list from which they can be then
transferred to the second list (and of course remember them next time
the list is shown).

If the situation permits, use only one list and allow multiple selection in it.

A mockup would help to better understand the current situation.
