Lets say you've got 22 songs on the disc, thats 21 copies of the image
too many, imho. The cover.jpg way seems a lot more economical (and
easier, since you don't need a tag editor).
From the ID3v2 specification:
There is the possibility to put only a link to the image file by using
the 'MIME type' "-->" and having a complete URL [URL] instead of picture
data. The use of linked files should however be used sparingly since
there is the risk of separation of files.

So, although you may set a list of predefined file names to look for
(cover.jpg, art.jpg, etc...), you should first check the APIC frame and
look if there is an embedded image, or a link to it. That way, the user
can specify the image's file name and location himself.
