Ok.
It appears similar like you've described. After I click "add to thunderbird", I need to wait a while and close add on manager window (or tab) with Ctrl+W and then the pop up dialog has focus with Orca.
Flat review also usable to check whether the download has
completed and the popup appears.
Btw, how do I invoke this extension?

Hi,
the last time I tried installing via TB's addon search, all that happened was TB downloading the XPI file, which I later needed to install myself.
I don't know whether this is the expected behavior, or I did something wrong, but yeah.
As for from file installation, when you have the XPi file downloaded, all you need to do is to navigate to menu Tools/Plugins, click on Options for all plugins button and select Install from file.
Then you select the file of the plugin.
Now, you need to confirm a Thunderbird dialog asking you whether you want to add the addon to Thunderbird.
In fact, this might've popped up when you clicked the Add to Thunderbird link, I recommend checking that out.
Unfortunately, the only way to work with this dialog I'm aware of is using OCRDesktop:

You click on the Add button, and the installation is done.

I do not hear any feedback to know whether the installation proceed or not.
