The only accessible matrix client with VOIP capabilities is Element at this time.

1:1 calls are handled natively by the Element app and it's accessible on the web, on android and with the electron powered desktop app.

For audio video conferences with multiple participants Element embeds jitsi meet as a widget. Jitsi meet is improving in terms of accessibility and it's already very usable.
