And I have never been able to use laptop layout without feeling I was using this layout for the first time each time and switch back to desktop use.
I had a laptop which had to be used with laptop layout, its use
was really not comfortable for me even if I had to use it
everyday. I always had to use learning mode to find the gesture I
needed because I had not assimilated the laptop keyboard logic.
I am not sure the question is to know which is the better everyday but what could be the best when starting Orca for the first time.
IMO the best is to ask the user on the first start, as NVDA does.
But if it is not something which can be envisaged why not enabling every possible keys by default and let the user make the choice, it is not a problem for someone who has no Insert key if Insert is enabled as others keys which are usable on computer.

Why not adding a choice in the modifier key selection to have both Insert&shift keys as Orca key by default?
In this situation, it is easy to press Orca+space and select the best layout.
Or put caps lock/shift as Orca key by default.
