So does your orca does run with XDG_VTNR set to the VT number?
To make sure you could run
XDG_VTNR=1 orca --replace
(assuming your wayland session is running on VT 1)
If that still doesn't help, we'd need brltty logs to make sure what is
actually going on, with
log-level server
in brltty.conf
