Unfortunately I do not have a reliable way to reproduce this problem.

Usually happens when I am flat review mode on the terminal, but never when I want or need it! It may just be an impression, but it seems to happen when I'm doing something that requires a lot of windows swapping using alt + tab

I have tried several times to create a way to reproduce this problem but always to no avail.

I always use orca master

Are you using Orca master and latest accessibility stack?
