I am afraid this might really be related to pulseaudio.

For me it happens on a fresh install of pulseaudio. When plugging or unplugging headphones pulseaudio card profile is changed and initially it also affects the volume that it might become muted.

If you have multimedia keys, try to unmute or increase the volume blindly. After doing this it should no longer mute when plugging / unplugging headphones - at least I managed to make it work on two laptops recently.

It might be a Linux bug. I think the audio stack is messed up somehow. I also experience audio problems at the login screen. Using Ubuntu Mate 19.04, controlling the audio volume on the user login windows is impossible. It usually defaults to 50% or 75% until I reach the desktop, then I can turn it down.
