I can confirm this issue. We've hack around it with enabling auto-login into Lightdm. We've not tried to figure out where the issue comes from precisely but it should be related to PulseAudio, not Orca or Speech Dispatcher.

try raising the volume after plugging in or unplugging your head phones. If it's the bug I think it is, it has nothing to do with Orca or your TTS. It's somewhere in the audio stack.

I am not sure if trying to do an ALT Super S at the loggin screen will resolve this bug like it did when I first had ubuntu installed on this computer when I had to work out if I was at the loggin screen before turning on Orca.
