I've now also tackled the double-presentation. Please test.

Also, I *think* that I've fixed all the mouse review issues that you've
reported that I can fix (e.g. I cannot fix the title bar thing in Orca).
Is that correct?

Steps to reproduce:
1) Open a caja window
2) Open the File menu with the mouse
3) Move the mouse on the first item of the file menu called "New Tab"

Result: Orca says "back push button grayed" (the button behind the menu)

Expected result: Orca should say "New Tab Ctrl+T"
