This is something liblouis can handle to some extend.
So it appears there must be a way to communicate such emphasis details somehow.

As you will see in that documentation, you can select which text
attributes you want marked in braille and you can select which cell or
cells to use to underline attributes on the display. By default, no
underlining occurs.
