I know I'm not answering your question, and I don't know what VM
software you have, but I'm able to use my screen readers (Orca and NVDA)
in my virtual machines using VMware Player just fine. I also don't have
any trouble toggling Orca on and off as needed. If the hot key stop
working for some reason, I can just hit alt+f2 and run orca --replace to
get it started again.
I'm not saying this isn't a good feature request. I just offer the above
in case there's something else going on or any of this could be used as
a work around in the mean time.

2 - remote desktops and virtual machines... I cannot currently use
virtual machine's screenreader when orca is running, if the virtual
machine viewer is using a gtk window, disabling orca is possible but I
actually believe that after few such toggles something breaks and no
global desktop shortcuts like that work anymore,requiring to kill
gnome-settings-daemon, also disabling and enabling orca is supposed to
be slow on my computer especially when a vm is running.
