I would suggest working from the bottom up. Make sure that audio is working.
If you have a sound file, be sure that you can play it. Next,
Speech-Dispatcher needs to run correctly: try using spd_say from the shell
prompt to speak a message.

With all of the above operating correctly, Orca should be fine, but if not,
capture a debug log.

It's a matter of systematically finding and addressing the problem.
