I confirming this issue, and if I remember right, oldest time I already
reported a similar issue in Bugzilla, but this issue a customized
tutorial message related. I opened the report when Orca main window is
removed.

If I run for example orca --replace command, the previous active
gnome-terminal window is accessible only when I doing an ALT+TAB
keystroke. Try following testcase:
1. Launch gnome-terminal command when Orca running.
2. Try run orca --replace & command.
3. Try press few ENTER keys after Orca launched again. You not will be
hear the prompt.

A possible solution, but I am not sure yet working or not:
In Python, have a Wnck binding with possible manipulating active
windows. I use this Python winding my Orca window switcher Orca
customization script.
Possible simple enough querying the active window, and activate themain
window after Orca full initialized to active window get again focus. For
example, when I logged in GNOME 3.10 and Orca starts, Orca
modifier+Space keystroke works me only when I goto the desktop (because
only the desktop window is focusable).
