I was able to reproduce it with the KP Insert. So it is not just caps
lock. I was also able to reproduce it using Orca v3.3.3 (i.e. before all
of the refactoring of keystrokes and rolenames began).

I just pulled the latest at-spi2-core and at-spi2-atk, built and
installed both, logged out and logged back in and now I cannot reproduce
the problem. When you get a chance can you please try the same?
