I think the most reliable out-of-the-box accessibility I've yet had was
with Trisquel. 5.5 (based on Ubuntu 11.04, but free of all things
Unity) is out. If you get the I18N DVD edition, you will have a talking
live session, from which you can use the distro, and install, if you
like, with speech (in any of about 50 languages) for your whole
session. On booting your fresh install, you will be greeted with a
talking login screen (they use gdm, even though it's Gnome 3). If you
run Trisquel, you may want to upgrade orca and libreoffice. Trisquel's
Mozilla browser (abrowser) is version 11. If you want a gui mail
client, install thunderbird, since Trisquel ships with Evolution (which
does not work with orca anymore). Go to http://trisquel.info and see
for yourself.
If you wish an accessible session for Ubuntu 12.04, you can hit
'ctrl+s', when you hear the bongos at login time. You may have to do
this twice (about 30 seconds apart), but Orca will start talking, and
you will have the options of installing or trying Ubuntu. I remind you,
12.04 is not yet officially released.

Secondly, in Ubuntu 12.04, what is the process for logging into and
activating speech after an install with speech? When I boot up I get
no sound or speech even though I had speech during an accessible
install.
