I think from what you write, my observations are similar.  I also have
the Orca main window checked in Orca preferences and that instance
will not show up while alt-tabbing around or any of that until Orca is
restarted.  Once manually started or restarted with the --replace
option, then the Orca main window shows up and you can use Orca
commands to access preferences or shutdown, etc.

I don't know any of the gsettings variables involved here.
