As I remember when I had a look at XFCE and accessibility it in fact was 
better than you describe. I think there is a reason for your 
observations and I am not sure how much could be done by the orca 
developers, it may be more of an issue for the distribution developers 
to sort out.

Put shortly, I found the problem was ensuring that at-spi was started 
early enough. Any application (including panels, menus, etc)which is 
started before at-spi will not be accessible through at-spi. I think 
some users have got round this problem by modifying the startx script, 
however that only solves it when you use startx.

Another possible cause to your problems could be setting the GTK_MODULES 
variable.

I think both are distribution problems as the solutions can depend on 
how you start the X server (eg. startx, gdm, etc).

Regarding the issue with ensuring at-spi is started early enough, is 
there anything which could be done from the gnome accessibility side of 
things to help distributors?
