Hi,
If punctuation is set to all then you do indeed hear two full stops when
reviewing by line.
However this isn't Thunderbird specific and indeed, Orca is throwing in
full stops all over the place with punctuation set to all. I get them
when bringing up menus, on button labels... and all over the place where
none exist.
Something wrong here...

If punctuation level is set to most, you'll hear only one instance.

Confirmed.
In fact I didn't even need to create a new message. I got that
problem when reviewing your message below. the first two spaces
inbetween the words are not spoken (nothing is), only the third
space is spoken.
I have confirmed that this problem occurs when composing too.
