I think I may have spotted something similar. My observations seem to be
that things lock up generally (eg. I can't use the short cut to the
desktop ctrl+alt+d) until I exit orca (I do it by going to a text
console and typing "orca -q").

This might have been hinted to recently but not sure. On occasion,
I've been losing speech at what seems like random points. Most often
is when I exit a program like Nautilus. I just got this when I
pressed Ctrl-W to close an icon view of Nautilus. I've also gotten
these speech losses when closing a dialog. Unfortunately it isn't
consistant so cannot provide exact instructions to duplicate the
problem every time.
I'm using SVN pulls of Orca from about a week ago under a Debian Lenny
Testing system.
