Yes, that works fine.
However,
  spd-say Schaltfläche
gives the same result as within Orca.

So perhaps the problem is in speech-dispatcher?

It would appear so! :-(

What does the command:

locale

show for LC_CTYPE?  Is it a UTF-8 locale?

Yes, it's "de_DE.UTF-8".
