Probably neither.  That question in its present form is way too
vague.  If you have a complete distribution such as Debian, Ubuntu, or
Fedora/Red Hat, you have gnome already built and all you should need
to do is follow the installation procedures for that distribution.
Slackware does require more work because they don't include gnome
anymore.  For that, we download and install a "mini distro" of gnome
but in either case, the end result will be the same - a working gnome
environment including Orca.

Now unfortunately for some folks, problems have arizen with getting
speech to work but that is often worked out by exchanging some
questions and answers on the list.  If you're that new to Linux, you
might wanna "Kick the tires" so to speak by booting a Ubuntu live CD
where you can pretty much put the CD in and let her run.  Well, you
need to take a couple small side steps to get Orca to come up talking
.  I can't remember right now where instructions are for that live
CD.  Also, if you're beyond that part, try to look at 
http://live.gnome.org/Orca
to start reading.  Also, you might wanna play with the text console
environment to get your feet wet first and then move onto the GUI with
its additional complexities.  Just my thought.
